What This Assessment Looks At
Step 1 - Intake Questionnaires


Before the appointment, you’ll complete secure online forms and brief screening questionnaires so important background information is already available when we meet.
Process Steps




We then meet via telehealth for a 90-minute conversation where we review your history, explore emotional and behavioral factors affecting eating patterns, and discuss your experiences and expectations related to GLP-1 medication and weight changes. The goal is to help clarify factors that may influence your long-term success.
Step 2 - Assessment Session
Step 3 - Clinical Summary & Recommendations
After our meeting, you’ll receive a written clinical summary outlining key findings along with personalized recommendations. This summary can be shared with your medical provider, therapist, or other behavioral health professionals if you choose. It also gives you a clearer understanding of your current behavioral and emotional patterns so you can better determine how GLP-1 treatment may support you long term and what steps may increase your chances of maintaining progress.

• This service does not provide nutritional or diet planning guidance, which should be addressed with medical or nutrition professionals if needed.
• This evaluation does not determine eligibility for medication but instead helps identify behavioral and emotional factors that may influence long-term success.
